Wisconsin Licensure Process

Once you have made the decision to join Aurora Health Care, you will have a dedicated Specialist ready to help you through the process to ensure a smooth transition between signing and start date. The Specialist will assist you throughout the licensure and credentialing process and will be there to help you through your first years so that you can build and maintain a solid, patient centered practice.

It is essential that all practicing physicians be licensed by the State of Wisconsin Department of Regulation and Licensing.

The licensure process takes approximately 90-120 days so it's a good idea to start early. You can request a license application packet by calling the State of Wisconsin at 608-266-2812.

It may take up to three weeks for the state to mail the packet to a potential licensee. You may reduce the time by taking advantage of the state's website which contains all the required forms and an excellent explanation of the process. In addition, Wisconsin requires a test on its laws that you must pass as part of your application process.

Aurora Human Resources Application

Aurora Health Care utilizes a web-based application, which is the formal employment application. We also require an authorization form allowing Aurora to conduct a background check and obtain references. All documents need to be completed as part of the application process.

Credentialing Verification Service (CVS) Application for Aurora

The CVS application is required to verify your training and background. This information is required for all hospital applications, as well as for the insurance plans contracted with Aurora Health Care. This process typically takes a minimum of 90 days. Start completing this application as soon as you receive it.

Hospital Privilege Application

This application is specific to any hospitals where you may practice and will be sent by the medical staff office. Depending upon the number of hospitals where you may need privileges, there may be more than one application. This process may take up to 90 days to complete.

DEA Registration

You need to register with the Drug Enforcement Administration or update your practice address on your current certificate. Complete this process 14 to 30 days prior to starting. Visit their website now.
