Aurora Health Care was created around a single idea: There is a better way to provide health care. We find better ways continually through Care Management.
Aurora has implemented Care Management initiatives focused on preventive health and wellness, episodic care, chronic disease management, and topics covering the entire continuum of care. Our physician champions play an integral role on each of the initiative teams and in supporting implementation across the organization. Each of the initiatives offers providers new resources and support tools to assist them in daily practice. An example of this is the patient registry designed for the cholesterol management initiative, which assisted providers in outreach efforts that resulted in significantly lower LDL levels in patients with coronary artery disease. (Aurora was named an AMGA Acclaim Award Honoree for these efforts.)
Care Management is a focused, coordinated approach to delivering care across a broad range of service locations, by many different providers, at many different points in time. Care Management provides a better way for our health professionals to work together toward achieving optimal outcomes. It allows us to be proactive and to use proven best practices consistently, wherever care is provided.
Our efforts to date have significantly improved clinical outcomes in many areas, but we have much work yet to do. For quality initiatives to be effective, there must be broad collaboration and data sharing. We are committed to being transparent with our Care Management/Quality efforts and are sharing our results with the Centers for Medicare and Medicaid Services, the American Hospital Quality Initiative, the Joint Commission on Accreditation of Healthcare Organizations, the Leapfrog Group, and other national organizations and initiatives.
